Buy one, get the companion for free. Buy both?
Look at store, look at options to distribute.
Therefore needs to do #20 first.

Maybe make it free and add paid option for synchronizing? Is this even possible on apple watch?

A big amount of the apps in the store are free and have some in-app purchase option.

It seems that the in-app purchase option is only available via the iOS app. An all in one purchase may therefore be the better option.
