AG-18 should not be a living repository of registry entries. These tables should be on smpte-ra.org and could be referenced from AG-18 if needed, but perhaps deleting Annex D and E would be fine.

HTML doc updates

* A new version of AG 18 was created in
  #3 and merged
  but these changes were later wiped out when the doc was
  converted to HTML. This commit reinstates those changes.
* Small updates to better match the Word document
  (except where the Word document contained errors)
* Improve mark-up: e.g. mark-up more Terms in the prose
* Move listings of Private-use and Public-use entries under
  SMPTE Control to the Registers Website -- closes #8
