Data cleaning export columns and header for Caddisfly question type #1931

Currently if the user exports data using the raw data export with the Advanced settings turned on, he gets all questions in the header without the Flow internal question ID. Usually only with the name of the questions, and if the user created unique IDs for questions then with those as well.

However, Caddisfly questions are generated still with the Flow internal question IDs. These IDs should not be shown.

Secondly, if a Caddisfly question has a user created question ID, this is shown properly but also with the question name. In this case, only the user created question ID should be shown.
