Add Dock address to submission form

In order for Dock to reward contributors we need to be able to collect their Dock address.

Hey @mike-parkhill ,

the website embeds my instance of google form. I would suggest Dock team to create its own google form, so you have control over form and data. Then we will simply replace the embed (ìframe`) code. What do you think?

Or maybe simpler solution is that I will invite you as a collaborator to form I created. What do you prefer?

@robinbortlik - yeah, we'll probably want to create our own form. Likely would be more acceptable to users if the Google form is owned by us too.
