Create form for Organizations to request Resilience

There needs to be a form created for organizations to provide more information for how the Resilience application could be beneficial for them.

Description:
The top of the form on the left side should have a text box that asks for the organization's name.
The top of the form on the right side should have a text box that asks for the user's role.
Underneath the top text boxes, on the left side should be a text box that asks the user's name.
Underneath the top text boxes, on the right side should be a text box that asks for the user's e-mail.
Underneath the user name and e-mail text boxes, there will be a text area that takes up the entire length of the form that asks "What's the organization's main area of the focus? (purpose, demographic, geographic, ETC.).
Underneath that, there will be a text area that takes up the entire length of the form that asks "what are the top 3 issues you want to improve?"
Underneath that, there will be a text area that takes up the entire length of the form that asks "How did you hear about us?"
At the bottom of the form, there should be a submit button - Center Aligned

Can you add /get-resilience to the description as the target route then? I think thats straight forward enough
