[Sources] Icons missing for several providers

Several provider icons are failing to load.

yeah, I used Mechanical Turk to get images for a couple thousand providers, but theres still ~2k or so that are missing logos.

Once I add Athena health providers (~15k) this will be a much larger issue. TBH, I don't have a good solution for this. Either we continue paying users on Mechanical Turk to find (sometimes incorrect) logos, and/or we make a community effort to find logos for each provider.

There must be an easy website that can be stood up where we just gang up to knock it out whenever we're bored. If you had a screenful of missing icons, I could see myself sitting to down to knock 'em out an hour at a time. The 360care provider sites should be an easy win since so many are missing.

At least the 360care provider links have identifying marks in their names, unlike the Trinity Health sites I mentioned in #110
