This was causing servers to not change their ID in raft even after an upgrade to raft protocol 3. After more than one server in the cluster gets into this state, they can't leader-elect anymore

I also compared Nomad code to Consul, and brought in another check that Consul has to prevent config change updates when the leader is removing itself, for clusters smaller than 3 nodes.

Also includes documentation for how to upgrade to raft protocol 3
