Process placeholders when opened from Lectern

Not sure how possible this is, but it'd be great if the plugin could handle opening books from Lecterns so that variables such as %player_name% would be populated. Additionally, let the book always open to page 1 (as vanilla opens the book to whatever the last viewed page was).

Edit: Opening books from signs & item frames would be cool, but is secondary to this request.
