PR # on release notes should be links

It would be nice if the PR # on release notes were links - see https://napari.org/release/release_0_4_13.html

I can think of at least two approaches here:

1. do this in sphinx (e.g. using https://github.com/sloria/sphinx-issues)
2. do this on the front-end, in JS

I'm open to either - maybe (1) sphinx approach is best one to try first

One caveat is that some of those pr numbers don't map to actual pr's - for example: in https://napari.org/stable/release/release_0_1_0.html there is mention of a pr napari/napari#100 which I can't find on the repo. Any guidance on how to go about this?
