Prompt for migration name by default

from my experience, having named migrations helps a lot with keeping track of them and debugging if anything goes wrong. In my opinion, migrate:create should have a prompt for inputting the name rather than just it being an argument.

Here's the script I'm using for the moment for this if anyone finds it useful.
