D20 Modern - couple fixes by request

[vince-roll20]

• Fixed repeating custom skill button text to a readonly span to make clicking the button easier
• added a tooltip gear icon to help indicate when there is a form-fillable tooltip available.
• Updated some skill button text to show the custom name in use. Craft, Knowledge, Perform, Read/Write Lang, and Speak lang
• Changed default ability for the Swim Skill to Strength
• added a skill category label and highlight for multi-skills
• updated Profession skill to allow custom entry
