This add functionality for fluence from an MLC aperture sequence, where the MLC leaf positions are assumed to move linearly from the first aperture, to the second. The fluence generated by this movement is no longer semi-binary, but linear in the bixels where the MLC moves over:

The benefit of this is that fewer apertures may be required for increase dose accuracy, and increases accuracy for IMRT fluences.

Update to Documentation

It also includes updates to the documentation, with new sections for External Surfaces and Structures.
