This pull request adds two mapping tables for
chartevents. The first CSV filemeas_chartevents_maincontainsitemidtoomop_concept_idandomop_concept_codefor eitherLOINCorSNOMEDdepending on the concept. The second CSVmeas_chartevents_valuefile is forvalues, concepts that do not have anitemid. Thevaluesare also mapped toomop_concept_idandomop_concept_codefor eitherLOINCorSNOMED. Mapping codes were reviewed and verified by @xborrat.
