This pull request updates the lab results mapping to LOINC table. Compared to the old table, new columns were added as well as new mappings. The table was reviewed by @xborrat.

thanks @a-chahin and @xborrat , this looks good. @tompollard, and @alistairewj any thoughts on the current file mapping name/columns headers for MIMIC IV v2.0 before we merge? And anything we should be aware of given the recent modifications to the MIMIC data structure? (This csv adjusts for the inconsistent mappings of itemids and labels in MIMIC III and IV, https://github.com/MIT-LCP/mimic-building/issues/452). Further descriptions about the dlabitems_to_loinc mapping table could be found under the Common Vocabulary Mapping subheading on this page: https://github.com/MIT-LCP/northwestern_federated. We will be adding other covid related mappings to the table for the time period after Jan 1st 2020, after clarifying the terms with the BIDMC lab personnel and any terms that could not be mapped will be submitted across to LOINC, https://loinc.org/submissions/new-terms/.
