Use native fetch by default - this has the benefit of working on Vercel Edge / Cloudflare Workers without modification (will also be easier to drop Node 16)Also, remove dependency on
ReadStreamapi since this is Node only. It does make uploading files a little harder, but this should be ok if we have examples.
