A small fix which adds a window title to the osascript dialogs for username and password.
The password dialog might be the first thing the user sees (depending on erase-install parameters), and there was no easy way to discover what process exactly is asking for information. By adding a window title containing either the localized erase or the localized reinstall string, this should be easier now.
