Fix up disease_model.rst
Description
Changes and notes
The disease_model.rst tutorial was very out of date betweenline numbers not lining up and since the refactor/implementation of
the Component abc.
This PR also implements a bit more testcode to hopefully ensure
things don't get so out of date again (though nothing can help the line numbers).
Testing
tests pass, including doctest
