feature: add observation field in enrollment

Description

New functionality that has been added to the student enrollment form.

• The following changes were made:
  • Added a note field on the student enrollment form.
  • The remark field is optional and allows users to add any additional relevant information that is not present in the standard registration form.

This new functionality will provide users with the ability to add additional information that may be helpful to school administrators, such as food allergies, medical information, or special needs details.
