[docs] Restructure documentation folder and add example #143

The documentation was a flat directory structure which made it unclear what went where in the output. This now restructures the filestructure to match the heirarchy of pages.

We add an example for how to create new pybloqs classes and restructure the documentation folder to match the structure more closely.
