Review all console.* in server/src/

While technically all console messages are formatted in mozlog format, we aren't giving them proper tags and watching their formatting very well. We should look for any console.info/error/etc in server/src/ and try to format them better.

Note that console.log() is only used sparingly for talking with a user at the terminal, and probably those instances can be left alone.
