SVG was created in Inkscape, starting with this (thank you!) and adding a little customization using ArcGIS with some custom drawing. All labels and leader lines were converted to paths and 'sandwiched' between 2 identical layers of polygons representing the State and territory boundaries. The bottom poly layer is styled/colored with CSS and the top poly layer is transparent and clickable.The jvectorMap used (USMAP.js) was created using this svg to jvectorMap tool. Bad things happen in IE8 and below. They are probably fixable issues if you need IE8 support.
