Cleanup and rework the debug module (DM). The DM can now support up to 4 harts. However, this feature is not used yet. The modifications in this PR were highly "inspired" (😅) by the multi-hart DM by @NikLeberg (https://github.com/NikLeberg/neorv32_soc/blob/main/vhdl/neorv32_debug/neorv32_debug_dm_smp.vhd).
