Cannot add Azure provider because validate button is greyed out

Running latest master (9f3c401), I am unable to add an Azure provider via the UI. The issue seems to be the "validate" button, which remains greyed out even after filling out all of the necessary fields.

Seeing this in the console -

It's an angular.validators error
#4622 could be the reason for this issue.

@djberg96 Can you confirm if you are seeing the same error shown above in the browser console?
